long distance (5 miles) wireless communication

hi,
im working on a project that require long distance wire less
communication up to 5 miles to deliver voice traffic. the transmitter
side should be as small as possible with very low power requirements.
Im looking for a starting point for this work and specifically looking
for complete chips or modules which will do the job.
If any of you have an idea please let me know.

2) with or without a FDD license?
3) antenna size requirements?
